RELEASE OF LIABILITY WAIVER FORM FOR ACTIVITIES, CLASSES AND OTHER PROGRAMS SPONSORED BY THE “BRAZIL-MINNESOTA CHAMBER OF COMMERCE”

In consideration for allowing myself and child to participate in programs and other activities at or promoted

by the BRAZIL-MINNESOTA CHAMBER OF COMMERCE (‘BMCC’) and further in consideration of the BMCC allowing me and/or my child(ren) to enter and use the facilities owned, leased or otherwise provided by BMCC (the “Facilities”), and in order to complete and confirm my child(ren)’s registration to participate in any event

promoted by BMCC, I hereby affirm, acknowledge and agree to the following:

The undersigned on his or her behalf and behalf of such children, hereby fully and completely releases, discharges and holds harmless the BRAZIL-MINNESOTA CHAMBER OF COMMERCE, its directors, officers, employees, volunteers and agents (hereinafter referred to as "releasees") from all liability to the undersigned or such children and all his personal representatives, assigns, heirs, and next of kin for any loss or damage, and any claim or demands therefor on account of injury to the person or property or resulting in the death of the undersigned or such children whether caused by the negligence of the releasees or otherwise while the undersigned or such children is in, upon or about the premises therein or participating in any program promoted by the BMCC.

I assume all responsibility for my belongings, as well as any and all damages to my personal property, or any bodily injury that may occur to me or the minor and further, I assume responsibility for property damage and bodily injury that I or the minor may cause to others, in each case arising or resulting from, an incident from, incident to, or as a consequence of, participation in any program affiliated with the BMCC.

I, for myself (and the minor) and my (and the minor’s) heirs, executors, and administrators, release, indemnify, hold harmless from, and waive all claims, damages, and rights of action, present or future, arising from my child and my own attendance on any event promoted by BMCC which I (or the minor or either of our heirs, executors, or administrators) may now or hereafter have against BMCC, the cities and towns around the event, and/or the event’s sponsors, vendors, and volunteers, including but not limited to BMCC, its affiliates, predecessors, successors and assigns event venues, and the respective trustees, directors, managers, volunteer staff, members, shareholders, employees, representatives, and agents of any and all of the foregoing.

I grant full permission to BMCC to use my (and the minor’s) name, voice, and/or likeness in any medium, including broadcasts, telecasts, website, social media, advertising promotions, in-house publications, photographs, videos or other accounts of this event.

I hereby certify that I have read this document and I understand its content.
